Attention Kindergarten, 2nd, and 6th Grade Families!

Parents of Kindergarten, 2nd, and 6th Grade students: Please note that by May 15th, 2024 you must show proof that your child has seen the dentist within the past 18 months and submit their dental exam report to school. This is a requirement for all children in the State of Illinois. If the SMILE Illinois dentist examined your child, you have fulfilled this obligation.

Medical Forms and Medication

Vision and hearing screenings will be conducted in School District 54 as per the Illinois Department of Public Health rules and regulations.

Please make sure all health paperwork and/or medications are dropped off at Nerge.  You are welcome to stop by between 8:00 AM and 4:00 PM Monday-Friday, fax health forms to our office at 847-357-5778, or email our school nurse, Mrs. Kneip, at [email protected].         

PHYSICAL EXAMS required for Kindergarten, 6th grade, and new students: the exam must have been completed within one year prior to the first day of the school year.   

IMMUNIZATIONS 

  • Kindergarten and all students: DTaP, MMR, Polio, Hepatitis B and Varicella vaccine are required. 
  • 6th grade: Tdap is required.
  • 6Th grade: Meningococcal vaccine after the 11th birthday is required. 
EYE EXAMS required for Kindergarten and any student entering the Illinois school system for the first time.
DENTAL EXAMS required for Kindergarten, 2nd, and 6th grade students.
 

Attention 6th Grade Parents: Physicals Required for Junior High Sports 

School District 54 requires that any student who participates in a team OR intramural sport, (e.g. cross-country, basketball, wrestling, volleyball, cheer, poms, or track) must have a current, approved physical on file in the health office PRIOR to tryouts. The physical must be administered by a physician or nurse practitioner and include the parent section completed and signed. 

 

Since sports physicals are valid for 395 days from the date of the exam, parents of students who are interested in participating in any junior high sport are urged to schedule a physical exam during the summer months. These appointments fill up quickly! A summer check-up will allow the student to participate in any sport throughout the school year.

Please do not email them before this date as email is not monitored during the summer months. PLEASE MAKE A COPY OF THE PHYSICAL FOR YOUR OWN RECORDS BEFORE TURNING IT INTO THE SCHOOL
